Outreach

Mount Gould Hospital Outreach

This service provides Information and Advice for patients and carers at Mount Gould Hospital.  The service is provided to the following wards:

  • The Stroke Unit 
  • Skylark
  • Kingfisher
  • PLYM Unit

Patients can be visited whilst in hospital or when discharged from hospital care. 

Help can be given with completing application forms and assessing patients entitlement to disability benefits. 

Cystic Fibrosis Outreach at Derriford Hospital

Information and Advice also provides a benefits and general advice service to people, adults and children with Cystic Fibrosis at Derriford Hospital. This surgery approach continues to be a model of good practice for patients with long term conditions which could easily be extended to other patient groups.

Glenbourne Outreach

Two afternoons a week, one of our Caseworkers provides information and advice to patients on the Harford and Bridford wards at the Glenbourne unit.  This is often to patients that will shortly be discharged and need advice on housing, benefits and finances, before and after they leave the hospital.

The staff of these wards have stated they have more time to  concentrate on the patients well being whilst the caseworker provides vital information to the patient they need when they leave hospital.
