About Long-Term Condition Self-Management (LCSM)

Empowering people living with any long-term physical or low level mental health condition to better manage their condition and to achieve a better quality of life.

We run a self-management programme for anyone aged 18 or over who has any long term physical or low-level mental health condition(s), and would like to find new ways to manage them to lead to an improved quality of life.  It is self-referral and free to you as it is funded by the NHS.

We have 12 years experience of running self management courses for people living with a long-term health condition(s), We help people take more control over their health by understanding and managing their symptoms effectively, leading to an improved quality of life.
