About Long-term Condition Self-Management (LCSM)

Empowering people living with any long-term physical or low level mental health condition to better manage their condition and to achieve a better quality of life.

We run a self-management programme for anyone aged 18 or over who has any long term physical or low-level mental health condition(s), and would like to find new ways to manage them to lead to an improved quality of life.  It is self-referral and free to you as it is funded by the NHS.

We are closed on all Bank Holidays and the week between Christmas and New Year.

We have 12 years experience of running self management courses for people living with a long-term health condition(s), We help people take more control over their health by understanding and managing their symptoms effectively, leading to an improved quality of life. Coming on our courses has empowered many participants to become more confident and less anxious, visit their GP's less, take less time off work, have less pain and need to take fewer medicines.

People who have been on our self-management course have reported an increase in:

  • Control of their symptoms and condition
  • Feeling of well-being
  • Ability to cope better with their health and their lives

  • Exercise and relaxation
  • Quality of consultations with Doctors

They also reported a decrease in:

  • Pain and fatigue
  • Anxiety, depression and breathlessness
  • Visits to GP's
  • Outpatient visits
  • A&E admissions
  • Hospital admissions

What does it involve?

Groups of 8-14 participants living with a mix of different conditions meet over four weekly sessions of three and a half hours at 156 Mannamead Road, Plymouth, PL3 5QL and are led through a structured course by trained tutors many of whom are also living with long-term conditions themselves.

In addition to the course activities, participants benefit from meeting other people experiencing similar problems, sharing ideas and information, and also building strong support networks which often last beyond the four week course.
