To support the implementation and day-to-day delivery of the Plymouth Community Assist service by undertaking triage assessments for new referrals to provide information and/or signposting. Keep accurate and detailed records of work undertaken and carry a caseload of on-going referrals.
Context: Plymouth Community Assist (PCA) is a new service commissioned by both PCC (Plymouth City Council) and Devon CCG (Clinical Commissioning Group). PCA aims to provide a new ‘front door’ to Adult Social Care in Plymouth with multiple potential referral routes. The aim of the service is ultimately preventative and is achieved by supporting people to live independent healthy lives for as long as possible and reducing demand on both the Adult Social Care system and the NHS. The service will run 9-5 Monday to Friday.
PCA will deliver a broad-based information, advice and casework service through phone, digital and where appropriate limited outreach support in the community. Initial contact will be conducted through a ‘triage’ assessment to inform how to best meet needs. Then ensuring people have direct access to the most appropriate community-based services.
The project is based on action learning and will change and evolve at pace based on emerging best practice, management information and qualitative feedback from service users and partner agencies.
The service will be developed through a staged mobilisation process:
Phase 1 - receiving referrals from Adult Social Care Teams and NHS discharge services
Phase 2 - accepting direct telephone referrals from the public
Phase 3 - developing an interactive digital offer to members of the public
