We have 11 years experience of running accredited chronic disease self-management course for people living with a long-term health condition(s), We help people take more control over their health by understanding and managing their symptoms effectively, leading to an improved quality of life. What does it involve?
Groups of 8-14 participants living with a mix of different conditions meet over six weekly sessions of two and a half hours at 156 Mannamead Road, Plymouth, PL3 5QL and are led through a structured course by trained tutors who are also living with long-term conditions.
In addition to the course activities, participants benefit from meeting other people experiencing similar problems, sharing ideas and information, and also building strong support networks which often last beyond the six week course.
